Event Booking

1. The Mount Zion Apostolic Church of Canada must be named on the certificate as the legal entity. The Church's formal address is 6810 Professional Court, Mississauga Ontario L4V 1X6;
2. The certificate must clearly indicate the policy number, start and end date of the policy;
3. The certificate of insurance must be for a minimum coverage of $2,000,000 in general property and liability per occurrence;
4. If your certificate has an aggregate amount, the minimum that the Mount Zion Apostolic Church of Canada will accept is $5,000,000.

It is the responsibility of the applicant to provide proof of the event date.

Liability insurance will be provided no less than two (2) weeks prior to the event date.
